Immunization Information

As a reminder, 70 O.S. § 1210.191 requires the following:

The State Department of Education shall provide and ensure that each school district in this state provides, on the school district website and in any notice or publication provided to parents regarding immunization requests, the following information regarding immunization requirements for school attendance: For school enrollment, a parent or guardian shall provide one of the following:

1. Current, up-to-date immunization records; or
 
PLEASE NOTE: (Okla. Admin. Code § 310:535-1-2, which states: A child, through his parent or guardian, may apply for an exemption from this requirement by submitting a form to the Department. The school shall maintain a copy of the approved application in the child's records. All exemptions submitted prior to a student entering 7th grade shall expire at the end of the student's 6th grade year. A new exemption is required to be completed and submitted by the parent or guardian prior to enrolling the child in 7th grade.)

Vaccines required to enter school (K-12)

  • Hepatitis B.

  • Diphtheria, tetanus, pertussis (DTap).*

  • Inactivated poliovirus (IPV).*

  • Measles, mumps, rubella (MMR).*

  • Varicella (chickenpox).*

  • Tetanus, diphtheria, pertussis (Tdap).**

*Students 4 through 6 years of age are required to have their final doses of DTaP, IPV, MMR, and Varicella prior to kindergarten entry.

**Students are required to have Tdap prior to entry into 7th grade.

The Oklahoma Department of Health now provides direct access to the vaccine record system. Below is a step-by-step guide to access your/your child's vaccine records that are entered at the health department.

 
2. Enter the fields requested and click "Search".
NOTE: The phone number or email must have been previously provided the last time the patient received vaccines and entered by staff or you will receive an error stating the record cannot be found. If you receive this error, you can call 405-632-6688 for assistance in obtaining your record request. 
 
3. If you record is found, you must click to send a one-time code to the phone number or email you provided.
 
4. Enter your code and click "VERIFY" to access your online portal account.
 
5. You will now be able to view and download your shot records online.
